Responsibilities
serves as a primary liaison between the Carle Health organization and external clinical partners
supports EpicCare Link and Community Connect initiatives
manages contracting of all new EpicCare Link sites (clinical, SNFs and ALFs)
collaborates with cooperative nursing and leadership, external providers, internal providers, IT-EHR, Business Development, Social Work, Radiology, and Health Information Management to support external partners
leads, manages and/or establishes Regional Work Group meetings across all regions
supports ongoing reporting requirements for Community Connect partners
validates HIPPA training has been completed for all new SNFs and ALFs
develops and maintains knowledge base of current Carle provider specialists and the breadth and depth of Carle specialty services
