Responsibilities
Provide case management services to participants in a program serving episodically homeless individuals with relapse histories.
Perform home visits & provide group facilitation.
Ensure adherence to policies and protocols for programs serving chronically homeless individuals.
Develop regular program schedule including activities, meetings, meals, etc.
Conduct initial service needs assessment with all clients entering the program.
Develop Individual Service Plan with each participant within three days of entry into the program.
Maintain daily contact with clients actively involved in the program.
Maintain appropriate files and case notes for each participant.
Provide crisis intervention (24-hour cell phone coverage).
Requirements
Bachelor’s Degree in related field.
May be substituted with a minimum of 5 years working with the target population.
Substance abuse experience required.
Excellent interpersonal and organizational skills.
Previous experience with group facilitation preferred.
