About the role
- RN Case Manager delivering high quality nursing care in hospice environments. Coordinating with interdisciplinary teams and developing care plans for patients in Denver area.
Responsibilities
- Administer high quality nursing care for patients of all ages in their place of residence
- Coordinate care with the interdisciplinary hospice group, patient, family and referring agency
- Assign responsibility for coordination of care of patients
- Determine scope and frequency of services needed based on the acuity of patients
- Assess the ability of other members of the care team to meet patients’ immediate needs upon admission and throughout care
- Develop & implement a plan of care to ensure quality and continuity of care
- Apply concepts of infection control and standard precautions to prevent contamination and transmission of disease
- Provide clinical directions to the Hospice Aide and LPN/LVN
- Prepare clinical and progress notes that demonstrate progress toward established goals
- Ensure continuity of quality patient care delivered with appropriate documentation
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current registered nurse (RN) license in the state of practice
- At least two years of experience as a Registered Nurse (RN) in a clinical care setting, with experience in hospice required
- Exceptional nursing skills as defined by generally accepted standards of practice
- Excellent interpersonal skills
- Experience and specialized skills in palliation and end-of-life care
- Ability to read, write, and comprehend English
