Responsibilities
Provides timely, client-focused administrative support to facilitate smooth service delivery across the program
Contributes to efficient referral processing, document management and general administrative tasks that support effective patient flow and quality outcomes
Responsible for client focused administrative support for all incoming referrals received by the Post Acute Care Department
A central point of contact for clients, families/carers, internal and external service providers who require access to the Post Acute Care program
Responsible for supporting the Post Acute Care Clinicians in managing incoming and outgoing referrals across a number of data platforms
Required to be tech savvy working across a number of digital systems, data platforms, and service coordination tools
Requirements
Previous health administration experience
Demonstrated ability to communicate with people at all levels and varying communication styles
The ability to work in collaboratively within a small team
A proven ability to multitask, manage competing priorities and problem solve
Proven computer literacy including the use of Microsoft Office applications (Word, Excel, Outlook, TEAMS) and ability to learn specialist programs
