Responsibilities
Assumes responsibility for the collection, organization, analysis, and dissemination of information on medication utilization
Integrate activities with clinical, safety, procurement, prescribing, administration, and distribution systems to optimize medication use
Involved in formulary management, medication policy compliance, and Pharmacy and Therapeutics Committee initiatives
Maintains the formulary
Evaluates pharmacoeconomic impact of pharmacy policy
Assures continual regulatory compliance
Educates staff, students, patients, providers and other health care professionals
Participates in professional development and continuing education
Coordinates drug shortage management strategies
Support change management related to formulary changes including communication plans, education and support
Adapts behavior to specific patient population.
Requirements
School/College of Pharmacy degree
PGY2 Residency in Drug Information or Informatics preferred
1 year experience in Drug Information, Informatics or similar field
License to Practice Pharmacy in the state of practice must be obtained within 30 days of hire
Must have computer skills and dexterity required for data entry and retrieval of patient information
Must be proficient with Windows-style applications and keyboard.
