About the role
- Registered Nurse Clinical Care Manager in Massachusetts focusing on care management for complex medical and social needs. Engaging with enrollees and coordinating with healthcare providers for improved health outcomes.
Responsibilities
- Engage with enrollee in their homes for effective care management
- Liaise between healthcare providers and enrollees for seamless communication
- Perform assessments including Comprehensive Assessment and Crisis Assessments
- Develop and implement care plans with routine updates
- Lead the interdisciplinary care team (ICT) to create holistic care plans
- Assist members in accessing community resources including housing and transportation
- Educate members about benefits and chronic condition management
- Follow up with members post-hospitalization for continuity of care
- Report abuse or neglect as mandated
Requirements
- Associates of Science (A.S) degree in nursing or Master's degree in social work or mental health counseling with independent license
- Three (3) years of experience as a Registered Nurse/BH Clinician or One (1) year as a Registered Nurse/BH Clinician with two (2) years of experience working with people with complex needs
- Intermediate proficiency with Microsoft Office, including Outlook, Word and Excel
- Strong interpersonal and communication skills
- Current unrestricted clinical license in the Commonwealth of Massachusetts as a Registered Nurse (RN) or relevant license
