Clinical Consultant Pharmacist enhancing health through personalized pharmacy services for long-term care. Collaborating with onsite physicians and nursing staff for optimal medication management.
Responsibilities
Act as a liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support.
Travel to LTC homes and retirement communities.
Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships.
Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy.
Complete all required documentation and billing processes as required by MOHLTC and College of Pharmacists of British Columbia.
Provide education to Residents and staff on various health-related topics including the Meds Check program and attend community-based wellness fairs and other events as required.
Conduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice.
Support CareRx Pharmacy policies and procedures within retirement communities and long-term care homes.
Complete medication audits on-site in LTC Homes and Retirement Communities as required.
Ensure regular on-site visits are made to all assigned clients.
Attend/participate in annual professional conferences and trade shows, when applicable.
Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable.
Help with pharmacy operations/drug distribution when required.
Attend and actively participate in clinical team meetings.
Requirements
Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.
Proven ability to foster strong partnerships with other members of the healthcare team.
Effective organization, planning, and time management skills.
Knowledge of Kroll Windows application and Microsoft Office programs.
Complete understanding of provincial regulations governing LTC homes and Retirement Communities.
Familiar with the provincial standards of practice established for LTC pharmacies.
Excellent customer service and interpersonal skills.

Skilled in critical thought processes and able to prioritize appropriately.
