About the role

  • RN Case Manager coordinating nursing care for patients in Western North Carolina. Instructing and supervising staff while managing caseloads and assessing patients' health.

Responsibilities

  • Coordinates the nursing plan of care
  • Provides direct nursing care
  • Instructs/supervises the LPN and Hospice Aide staff in the delivery of nursing care
  • Manages the nursing care for an assigned caseload of approximately 15 patients
  • Assesses the patient's physical and mental health
  • Records and reports the results of the nursing assessment
  • Plans, initiates, delivers, and evaluates appropriate nursing acts
  • Collaborates with other health care providers in determining the appropriate patient and family-centered plan
  • Reports and records the plan for care, nursing care given, and the patient's response to that care
  • Supervise, teach, and evaluate those who perform nursing functions

Requirements

  • Two years of clinical experience preferred
  • Valid NC RN license
  • Valid Driver’s License and proof of automobile insurance
