Responsibilities
- Perform face-to-face and telephonic assessments with members with substance use disorders (SUD)
- Provide integrated care to ensure members receive timely, high-quality, and coordinated services
- Engage members in their own communities and build trust through meaningful care coordination
- Complete required assessments, including the Comprehensive Risk Assessment (CHRA)
- Coordinate behavioral health, substance use, and medical services
- Oversee care planning and transitions while facilitating interdisciplinary care team meetings
- Collaborate with internal departments and community organizations to create a culturally competent care experience
Requirements
- Virginia licensed RN or LMHP
- Minimum of 1 year of experience working directly with individuals with substance use disorder (SUD)
- Case management experience providing care transitions for ASAM levels of care
- Ability to travel to region-based facilities and homes for face-to-face assessments
- Exceptional oral and written communication and interpersonal skills
- Ability to work with minimal supervision
- Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel
- Valid driver's license, car insurance, and reliable transportation
