Responsibilities
- Conduct comprehensive assessments to identify clients' strengths and needs, including substance misuse issues, mental health concerns, housing barriers, and other challenges affecting their well-being.
- Develop individualized care plans that address clients' specific needs, goals, and preferences.
- Connect clients with appropriate community resources, such as substance abuse treatment programs, mental health services, medical care, and vocational training.
- Process rental assistance support payments.
- Serve as a strong advocate for clients, ensuring their rights and needs are respected and met.
- Offer ongoing emotional support and guidance to clients, helping them navigate challenges, build resilience, and develop healthy coping mechanisms.
- Regularly assess clients' progress towards their goals, adjusting care plans as needed.
- Facilitate access to community resources, social activities, and peer support networks to promote clients' integration into their new housing and broader community.
Requirements
- 1-3 years of experience working with individuals experiencing homelessness, substance misuse, or mental health challenges.
- Knowledge of trauma-informed care, harm reduction principles, and evidence-based practices in substance abuse treatment and mental health.
- Familiarity with community resources, housing programs, and support services for homeless individuals.
- Ability to work under pressure and respond to deadlines without sacrificing quality.
- Ability to demonstrate a steadfast commitment to fostering diversity, equity, and inclusion within the workplace.
- Excellent communication and interpersonal skills, with the ability to establish rapport and trust with clients from diverse backgrounds.
- Strong organizational and time management abilities to effectively handle a caseload and meet deadlines.
