About the role
- Registered Nurse Case Manager providing comprehensive care coordination in Arizona. Responsible for assessing patient care plans and facilitating health services for optimal patient outcomes.
Responsibilities
- Provide comprehensive care coordination for patients as assigned
- Assess and develop, implement, monitor and document utilization of resources and progress of the patient through their care
- Facilitate options and services to meet patients' health care needs
- Manage individual patients across the health care continuum
- Act in a leadership function for process improvement activities
- Communicate interdisciplinary patient discharge plans
- Evaluate medical necessity and appropriateness of care
- Educate internal members of the health care team on case management concepts
Requirements
- Knowledge of case management or utilization review (bachelor's degree in case management or health care)
- Current Registered Nurse (R.N.) license in state worked
- Basic Life Support (BLS) certification required in acute care settings
- 3-5 years clinical experience
- Working knowledge of care management, acute care and/or home care environments
- Critical thinking and problem-solving abilities
- Effective communication skills
- Time management skills
- Ability to work effectively in an interdisciplinary team format
- Experience in an acute care hospital and/or home care setting within the past 12 months as a Case Manager in specialty area
