About the role
- Case Manager in Mamaway Wiidokdaadwin supporting Indigenous clients' health and social needs. Coordinating resources and building rapport for holistic health services in the community.
Responsibilities
- Maintain a time-limited and goals focused case load from internal referrals sent by Mamaway Wiidokdaadwin clinicians and team members
- Screen clients for eligibility in case management through MWIIPCT
- Perform a comprehensive assessment of a client’s social and community needs as they relate to a case management service
- Develop rapport, trust and ethical relationships with clients and stakeholders
- Comply with all relevant standards, policies, and legislation
- Demonstrate respect for client’s individuality and autonomy
- Performs other duties as assigned.
Requirements
- Completion of a Social Service Worker Program and 2 years experience; or an equivalent combination of education and experience.
- Completion of training in case management considered an asset
- Demonstrated knowledge of health, social and community-based resources required to provide case management services
- Communication skills to explain the case management process to clients and collaborators
