Requirements
- Must reside in the state of Kentucky or in a county that is contiguous to the Kentucky state border in Ohio, Indiana, Illinois, Missouri, Tennessee, Virginia, or West Virginia.
- Ability to travel to member's residence within 30 to 40 miles in the state of Kentucky.
- An active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
- 1+ years of maternal child nursing experience such as labor and delivery, neonatal ICU, postpartum/nursery.
- Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
- Knowledge of community health and social service agencies and additional community resources.
- Preferred Qualifications: BSN, 3+ years of in-home assessment and care coordination experience, experience with health promotion, coaching and wellness, previous managed care or Medicaid health plan experience, Certification in Case Management (CCM).
