About the role
- CAP Case Manager supporting the Community Alternatives Program for Disabled Adults and Children at Healthkeeperz. Conducting assessments, care planning, and service coordination with individuals in their homes.
Responsibilities
- Conduct assessments and determine CAP eligibility
- Develop individualized care plans based on identified needs
- Coordinate services with families, providers, and community agencies
- Perform home visits and follow-up monitoring
- Maintain accurate documentation in E-CAP; review aide notes; support compliant billing
- Ensure compliance with CAP and state program guidelines
- Participate in required training and continuing education
Requirements
- Bachelor’s in Social Work + 1 year related experience
- Bachelor’s in Human Services + 2 years related experience
- Bachelor’s in Non-Human Services + 2 years related experience
- Current NC RN license + 1 year case management experience
- Apprentice/Trainee (Bachelor’s or RN without full experience) – requires ongoing supervision
- Knowledge of casework, care coordination, and social services practices
- Familiarity with community resources
- Strong communication, time management, and organization
- Ability to travel for visits and meetings
- Proficiency with electronic documentation and virtual communication tools
