- Provide ongoing assessment, oversight, and advocacy for clients to ensure needs are being addressed.
- Provide timely and thorough written reports and documentation of billable services.
- Collaborate with the Care Management team for professional development and support for department work processes.
Requirements
- Licensed MSW, MFT, MA Gerontology or related field highly desirable (minimum two years of experience)
- In-depth knowledge of the aging process, conditions affecting older adults, and care management best practices
- Excellent communication skills (written and verbal), attention to detail, and time management abilities
- Integrity, honesty, and commitment to high standards of excellence and professional ethics
- Ability to work collaboratively with a multidisciplinary team
- Availability all weekdays and some evening/weekend on-call rotation
- Valid California driver's license, insurance, and personal transportation
