Case Manager role providing one-on-one life skills development for housing support program participants. Collaborating with treatment teams to ensure successful transitions to self-sufficiency.
Responsibilities
Provide one-to-one life skill development for HSP eligible participants
Interventions designed to improve client’s social and community competencies through building or reinforcing daily living skills and budgeting that will enhance the client’s ability to successfully locate and maintain housing in their community
Participate in treatment team meetings and agency specific case staffing
Prepare required written documentation, including case notes, incident reports, & agency forms
Participate in agency training, unit/staff meetings, activities, & events
Requirements
Associate degree preferred, Bachelor's degree in Social Work, Psychology, or related field preferred
One (1) year of direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, facilitates and/or related community-based organizations, preferred
Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools
Excellent time management, organizational and multi-tasking skills
High level of accuracy and attention to detail
Ability to maintain discretion and confidentiality with sensitive information
Bilingual Preferred
