Bilingual Spanish Care Manager providing long-term care management for economically disadvantaged seniors. Conducting assessments and coordinating care services throughout Los Angeles County.
Responsibilities
Provide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers.
Conduct in home and telephonic assessments, care planning, and client monitoring.
Coordinate linkages with community-based services and resources.
Maintain legal and clinical written record of each client’s case.
Ensure timely implementation of services for clients by completing all work within required timeframes.
Address client’s needs by developing and implementing a comprehensive care plan.
Ensure coordination of services and continuity of care by maintaining ongoing documentation.
Maintain effective communication with other disciplines working with clients.
Increase knowledge of care planner functions and actively participate in supervision.
Requirements
Bachelor's Degree in Social Work, Psychology, Gerontology or related field required.
Master's Degree in Social Work (MSW) preferred.
BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.)
1-2 years experience working with the elderly.
1+ year experience working in a hospital, home health agency or related setting in which the primary targeted population is older adults.
Ability to travel within program’s catchment area and visits to client homes is required.
Strong organizational, verbal and written communication skills; Demonstrated customer focus; and Ability to work well with others required.
Travel 50+% of the time in Los Angeles and surrounding areas.
Must have a valid driver’s license, automobile insurance and reliable transportation.
