Responsibilities
- Deliver virtual primary and preventive care to underserved patients across Texas
- Manage chronic conditions, acute concerns, and routine follow-ups through telemedicine
- Build trusted relationships with patients who have limited access to consistent medical care
- Support and refine clinical programs focused on hypertension, diabetes, medication adherence, and reducing preventable hospitalizations
- Partner with care coordinators, CHWs, and RNs to close care gaps and ensure seamless follow-up
- Provide clinical guidance during case reviews and escalations
- Collaborate with product and operations teams to improve care delivery workflows

Requirements
- 2+ years of clinical experience, preferably serving medically or socially complex populations
- Active Texas NP license
- Bilingual - Spanish and English fluency required
- Must be physically located in Travis County (Austin,TX) with access to a quiet, HIPAA-compliant, internet-connected workspace
- Experience supporting patients with chronic disease, behavioral health needs, SUD, homelessness, or Medicaid coverage
- Comfort working within a multidisciplinary care model
- Board certification (or eligibility) in Family Medicine
- Comfortable delivering telemedicine and working across multiple digital platforms
- Ability to care for patients across the lifespan (pediatrics to geriatrics)
- Familiarity with FQHCs, safety-net care, or Medicaid populations preferred
- Startup or fast-paced environment experience is a plus
