About the role
- Case Manager RN providing nursing care for home health patients in Washington's Olympic Peninsula. Supervising LPNs and HHAs while promoting continuity of care in various settings.
Responsibilities
- Assesses, plans, implements, coordinates and evaluates the nursing care of patients in their place of residence
- Promotes continuity of care for patients on their assigned caseload
- Provides direct patient care and supervises the care provided by LPNs and HHAs
- Works in collaboration with all home health disciplines assigned to provide care for patients on caseload
- Applies principles of learning in teaching patient’s self-management and methods to regain independence
- Provides on-call support as assigned for patients
Requirements
- Associate’s degree required
- B.S.N. preferred
- Minimum of one year recent experience preferred in home health, acute or long-term care
- Current Washington State Registered Nurse license, required
- Current BLS certification, required
- Valid Washington driver’s license, required
- Proof of insurance on automobile used for work, required
- Must have computer internet connectivity (OMHH work laptop provided)
- Must have reliable automobile transportation for entire scheduled shift to travel to patient residences
