About the role
- Physical Therapist providing rehabilitation services to patients in their home. Ensuring quality care through physician-directed plans and evaluating patient needs in home health and hospice.
Responsibilities
- Provides physical therapy services to patients according to a written physician's plan of care
- Assessing and evaluating therapeutic/rehabilitative/functional status
- Directing physical therapy treatment
- Instructing patients and families/caregivers in the use and care of therapeutic appliances
- Reporting to physician patient’s reaction to treatment or changes in condition
- Prepares and submits clinical and progress summaries based on the attainment of goals
Requirements
- PT Licensed in the State of Idaho
- Must have a current CPR
- Driver's License and Auto Insurance
