About the role
- Behavioral Health Crisis Care Manager responsible for assessments and crisis intervention for patients. Collaborating with healthcare providers to create individualized care plans and support patients during acute crises.
Responsibilities
- Conduct thorough assessments to evaluate clients' mental health status, behavioral issues, and psychosocial needs
- Collaborate with patients, families, and healthcare providers to develop individualized care plans
- Communicate and collaborate with the patient care team to coordinate patient care
- Provide immediate crisis intervention including de-escalation techniques and counseling
- Educate and provide guidance to patients about mental health conditions and available resources
- Facilitate referrals to appropriate inpatient or outpatient mental health services
- Maintain accurate and timely documentation of patient assessments and treatment plans
- Advocate for patient's needs and rights in a culturally competent manner
Requirements
- Must have a Masters in Social Work, Psychology, Counseling, or related field from an accredited institution
- 2 years recent experience in behavioral health setting preferred
- 3 years recent healthcare experience preferred
- Experience in Acute Care setting preferred
- LCSW or LPC license in the state of Georgia required
- Must have working knowledge of software/EMR applications
- ACM, CCM preferred
