Case Manager in Housing Support Program helping homeless families transition to self-sufficiency. Providing life skills and case management services to support clients' needs.
Responsibilities
Interventions designed to improve client’s social and community competencies through building or reinforcing daily living skills and budgeting that will enhance the client’s ability to successfully locate and maintain housing in their community
Ability to work with clients with severe emotional needs in diverse settings.
Participate in treatment team meetings and agency specific case staffing
Prepare required written documentation, including case notes, incident reports, & agency forms
Participate in agency training, unit/staff meetings, activities, & events
Other duties and projects as assigned.
Requirements
Associate degree preferred, Bachelor's degree in Social Work, Psychology, or a related field preferred
One (1) year of direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, facilitates and/or related community-based organizations, preferred
Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools
Excellent communication skills (verbal, written, and interpersonal)
Excellent time management, organizational and multi-tasking skills
High level of accuracy and attention to detail
Ability to maintain discretion and confidentiality with sensitive information
