About the role
- Clinical Team Lead providing clinical oversight and support in residential settings for individuals with intellectual and developmental disabilities, ensuring high-quality care and staff supervision.
Responsibilities
- Provide clinical oversight, staff supervision, and operational support across residential homes
- Conduct initial nursing assessments and ongoing reassessments for clients
- Review, monitor, and ensure accuracy of Medication Administration Records (MARs)
- Manage medication reorders and coordinate with pharmacies
- Monitor clients for changes in condition and report concerns appropriately
- Accompany residents to medical appointments and communicate follow-up care plans
- Supervise field staff across residential homes
- Train, onboard, and mentor new hires
- Conduct competency evaluations and skills validations
- Perform routine site visits to ensure compliance with state regulations and agency policies
- Provide coverage for call-offs and staffing shortages when needed
Requirements
- Active LPN license in good standing
- Minimum 2 years of nursing experience with IDD population and/or residential care settings
- Strong knowledge of medication administration and documentation
- Proven supervisory or leadership experience
- Reliable transportation and ability to travel between multiple locations
- Flexible availability, including evenings and occasional weekends
